August JSK Sign & Nametag form
Sign and Name Tag forms are due no later than August 12th. If this information is not turned in by August 12th, you can expect the following:
- Your Booth Sign will read as it appears on your contract
- Name tags can be picked up at check-in.
If you are participating in both shows you do not need to fill this out twice.
